Governed action detail records an admitted, refused or held operation and its authority context. It includes the semantic request, digest, mission and outcome evidence.

Controls and behaviour
The record is opened with its mission context. Its fields distinguish admission state, provider outcome and effect verdict. Back to mission activity returns to the relevant mission's call list.
Access and limits
A provider outcome of unknown keeps equivalent writes blocked. A verified effect can arrive later and update mission completion. An action digest binds an exact request and adapter version, not a general permission to act.
